Thank you for reporting your potential exposure. Please review the following information and submit the questionnaire below so that we can help you make an informed decision.

  • The CDC recommends that you quarantine if you have been in close contact with someone who has COVID-19.

  • Close contact is defined as being within 6 feet of someone for a cumulative total of 15 minutes or more over a 24-hour period.

  • People who are fully vaccinated do NOT need to quarantine after contact with someone who had COVID-19 unless they have symptoms. However, fully vaccinated people should consider getting tested 3-5 days after their exposure, even if they don’t have symptoms. Until they receive a negative COVID test, they should strongly consider wearing a mask indoors in public for 14 days following exposure or until their test result is negative.

  • People who have tested positive for COVID-19 within the past 3 months and recovered, as long as they do not develop new symptoms, do not need to get tested. If this applies to you, please denote in the comments field below.
